Client Intake Paperwork

For New Clients, Please fill out these forms at least ONE day before your appointment.
After you book your first appointment, you will receive an email from our Client Portal (Be Replenished Counseling).

You will create an account AND then use the FORMS tab at the top inside your new account to SIGN the INTAKE PAPERWORK.
​
This must be completed at least one day before your appointment.
